Final Forms (Grades 5th-12th)

 

My name is Patti Tremblay and I serve as the Final Forms Administrator. Final Forms is an online platform that GCA utilizes to manage and store required paperwork for both academics and athletics. This program will allow you to sign these forms online. Every GCA student grades 5th-12th is given a Final Forms account.

 

 

If you haven't already, please confirm your account through the email that you received from Final Forms. (You will receive an email by the end of May) We use parent emails that were provided during the admissions process. If you would like to use a different email address, you will have the opportunity to change your email address once confirming your account. If your student already has a Final Forms account from another school, unfortunately, we cannot transfer that profile to GCA for privacy purposes. You will need to set up a new GCA profile.

 

 

Students in 5th-12th grade will be given a GCA email address. Please click HERE for instructions on how your student is to access their account.

 

 

Final Forms registration for the 2023-204 school year will be open at the end of May. At that point, you will be able to log in and sign the required paperwork for participation in GCA Athletics (please note that there will be some additional academic forms that will require your attention closer to the beginning of the school year). You have the opportunity to select sports that your student may be interested in playing. This is not a commitment to a team or a guarantee of making a team, it is simply an indication of interest and ensures that you do not miss important information regarding tryouts and meetings.

 

 

Your athlete will need an up-to-date sports physical before participating with any GCA TEAM or summer workouts. You are welcome to turn those in now. Please send the original to the Lower Campus front office attention Patti Tremblay and she will ensure it is recorded in Final Forms. You may also email a SCANNED copy to [email protected] (our apologies but we cannot accept a picture it must be scanned and attached to an email.) Also, please keep in mind if you get your physical done on April 15 or later it is good for the entire 2023-2024 school year.
